Position Description

Job Title: Director of Health & Wellness 

Position Summary:
We are seeking a dedicated and compassionate nursing professional to join our team. In the Director of Health & Wellness role, you will be responsible for planning, implementing, and evaluating resident care while ensuring compliance with all state and federal regulations. 

Key Responsibilities:

  • Provide nursing services in accordance with state and federal regulations.
  • Conduct pre-admission assessments for potential residents.
  • Maintain compliance with personnel policies and community procedures.
  • Assist in emergency preparedness and evacuation procedures.
  • Perform resident assessments and update care plans as needed.
  • Assist residents with scheduling medical, dental, and laboratory appointments.
  • Administer and document medications, ensuring adherence to the five rights of medication administration.
  • Provide medical treatments as ordered by physicians.
  • Monitor and support restorative nursing programs.
  • Observe residents and report changes in condition to appropriate personnel.
  • Communicate effectively with residents, families, visitors, and healthcare team members.
  • Assist residents with nutritional and hydration needs.
  • Maintain a clean, safe, and homelike environment for residents.
  • Respond promptly to resident call lights and emergency calls from Independent Living.
  • Participate in staff training, quality assurance, and risk management programs.
  • Maintain confidentiality of resident health information.
  • Perform additional duties as assigned by the Supervisor.

Supervisory Responsibilities:

  • Reports to the Executive Director and operates with a high level of independence.
  • Hires, trains, supervises, and evaluates nursing staff.
  • Conducts performance reviews and takes disciplinary actions as necessary.

Qualifications:

  • LPN license required.
  • At least 5 years of experience in long-term care or a related field.
  • Combination of education and/or experience equivalent to graduation from an accredited college or university, with major work in related field.

Skills & Abilities:

  • Strong communication and interpersonal skills.
  • Ability to work compassionately with elderly residents.
  • Knowledge of nursing practices, senior care regulations, and medical procedures.
  • Strong leadership and motivational abilities.

Physical Requirements:

  • Ability to sit, stand, walk, bend, stoop, kneel, and reach throughout the workday.
  • Ability to lift up to 25 pounds unassisted.
  • Ability to assist in resident transfers using appropriate equipment.
  • Must have normal eyesight (or corrective lenses), three-dimensional vision, and color recognition.
  • Must be able to hear, communicate clearly, and follow verbal instructions.

Work Environment:

  • Well-lit and ventilated facility with occasional exposure to dust, fumes, odors, and noise.
  • May encounter emotionally distressed residents or visitors.
  • Occasional weekend, evening, or extended hours required.
  • Potential exposure to communicable diseases.
